Understanding the ACL's Biomechanical Function
The ACL stabilizes the knee joint by preventing forward translation of the tibia (shin bone) relative to the femur (thigh bone) and resisting rotational torque during sudden pivots, jumps, or decelerations.
Phases of ACL Reconstruction Rehabilitation
Phase 1: Protection & Full Extension (Weeks 0–4)
Graft protection, full 0° knee extension restoration, pain management, and preventing hamstrings/quadriceps atrophy.
- Active patellar glides to maintain kneecap mobility.
- Prone hangs and towel extensions to achieve full 0° knee extension.
- Neuromuscular electrical stimulation (NMES) for quadriceps activation.
Phase 2: Single-Leg Strength & Proprioception (Weeks 5–12)
Building single-leg stance balance, eccentric hamstring strength, and squat mechanics up to 90 degrees.

Phase 3: Plyometrics & Agility (Months 4–6)

Phase 4: Return-to-Sport Testing (Months 7–9+)
Passing limb symmetry index (LSI > 90%), hop testing, and sport-specific agility clearance.
