Effective Physiotherapy Exercises for Lower Back Pain & Sciatica in Vasai

By Dr. Rashmita Karvir-Kekre (PT) Spine & Back Pain August 2026

Lower back pain is one of the leading causes of disability worldwide. Whether caused by prolonged desk work, improper lifting, degenerative disc disease, or a herniated lumbar disc causing sciatic nerve radiation, structured physical therapy offers a permanent, non-surgical path to recovery.

Understanding Sciatica vs. Mechanical Back Pain

While mechanical back pain usually presents as a localized dull ache across the lower lumbar spine, sciatica occurs when nerve roots (L4, L5, or S1) become compressed or irritated. This leads to sharp, burning pain, tingling, or weakness traveling down through the gluteal region, thigh, calf, and into the foot.

Common underlying triggers include:

  • Lumbar Disc Herniation or Bulge (Slip Disc)
  • Lumbar Spondylosis & Osteophyte formation
  • Piriformis Syndrome (muscle compression of sciatic nerve)
  • Sacroiliac (SI) Joint Dysfunction

Key Physiotherapy Exercises for Sciatica & Lower Back Pain

Before performing any exercise, an accurate clinical assessment by an experienced physiotherapist is crucial. At Health 360 Clinic in Vasai West, Dr. Rashmita Karvir-Kekre (PT) prescribes directional preference exercises based on the McKenzie method:

1. Press-Ups / Cobra Extensions (For Posterior Disc Bulges)

Lie face down with hands placed under your shoulders. Gently press up through your palms, extending your lower spine while keeping your pelvis relaxed on the mat. Hold for 5 seconds and repeat 10 times. This helps centralize radiating sciatic pain.

2. Nerve Gliding / Sciatic Flossing

Sit upright on a rigid chair. Slowly extend your affected knee while flexing your foot upward towards your shin while leaning your head slightly backward. This restores smooth mobility to the sciatic nerve pathway.

3. Bird-Dog Core Stabilization

Position yourself on all fours. Extend your right arm forward and left leg backward until parallel to the floor, maintaining a neutral spine without arching. Switch sides and repeat 10 times per side to strengthen deep lumbar stabilizers (transversus abdominis and multifidus).

4. Piriformis Stretch

Lie on your back, cross your affected leg ankle over the opposite knee, and pull your uninjured thigh gently towards your chest until a gentle stretch is felt in the buttocks.
